 About Us


Untitled design.jpg ​Palm Beach County is one of seven counties in Florida that have been identified as an area where HIV burden is highest. One of the goals of the Ending the HIV Epidemic initiative (EHE) is to reduce new HIV infections by 90% by 2030. To reach this lofty goal, Palm Beach County's EHE initiative takes a phased approach of infusing resources to the places that need them the most.

We do this through community outreach and engagement (CORE), rapid entry to care (REC), telehealth adherence (TAC) and other programs. Our case managers are out in the community meeting with clients and peers to ensure that medical care, medications and essential support services are provided to low-income people with HIV.

 Our Services

​Rapid Entry to Care

Rapid Entry to Care services provide an EHE client with low barrier medical services following a new HIV diagnosis or when reengaging in care.​

Community Outreach, Response & Engagement Services (CORE)

EHE's CORE team of case managers cover a range of client-centered activities that are focused on identifying people with HIV and linking those who are out of care into care. CORE services are a bridge to HIV treatment initiation through Rapid Entry to Care as well supportive services through EHE and the Ryan White Program.

Tele-Adherence Counseling (TAC)

Tele-adherence counselors provide adherence counseling in-person and via phone. Eligible clients receive access to a smartphone and a smartphone-based app that assists with managing medications and appointments, monitoring viral load and CD4 counts, as well as mood and stress levels.

Harm Reduction Intervention Services (HRIS)

The community-based Syringe Services Program uses evidence-based harm reduction strategies to idenitfy and link to care those people with HIV who inject substances.

Vocational Rehabilitation Services (VR)

Vocational Rehabilitation Services include training, coaching, linkage and related services designed to secure consistent and long-term employment for people with HIV whose HIV status or other disability, as defined by the Americans with Disabilities Act, are a barrier to emplyment.

Healthcare + Housing Opportunities (H2O)

Healthcare + Housing Opportunities provides eligible clients with access to a range of services intended to maximize access and adherence to comprehensive health care in coordination with stable, permanent housing for priority populations of people with HIV.

Please note that this program does not provide standalone housing assistance to clients, rather it provides opportunities that support upward economic mobility leading to self-sufficiency through increased engagement in health care and support services to obtain and maintain stable, permanent housing.

211 Transportation Services (211)

211 Transportation Services provides non-emergency transportation services that enables an eligible EHE client to access or be retained in core medical and support services.

Health Insurance Premiums (HIPS)

Health Insurance Premium Services provide financial assistance for eligible clients with HIV to maintain continuity of health insurance.
