___  Read and understand Statement of Collection of Social Security Numbers

___  Read and understand the Subordination Policy

___  Read and understand the Insurance/Mold Remediation Hold Harmless Affidavit

___  Copy of valid Driver's License or State Identification for ALL adult household members

___  Copy of only U.S. Birth Certifcates or valid U.S. Passport OR Naturalization Certificate.  Provide a valid permanent resident card for ALL Non-U.S. Citizen household members

___  Copy of most recent paystub for ALL working adult household members (OR) a current Verification of Employment (VOE) completed and signed by employer within 30 days of submission of application

___  Copy of most recent Social Security, Retirement and/or Disablility Award letter for ALL adult household members (if applicable)

___  Completed YTD Profit and Loss (P&L) Statement for ALL household members (if self-employed or current 1099 income earner).  P&L from your previous year taxes is NOT accpetable

___  If an adult student is employed - copy of class schedule on school's letterhead

___  If unemployed, a copy of unemployment benefits statement or a completed Unemployment Affidavit for ALL unemployed adult household members 

___  Wage Earner Statement from Social Security Administration for ALL unemployed adult household members

___  Court ordered child support documentation and/or proof from Palm Beach County Clerk and Comptroller's Office that there is/are no child support cases(s) or award(s) and ALL appropriate legal documentation for adoption, divorce, battered spouse and restraining order

___  Completed Verificiation of Deposit (VOD) for ALL current accounts (Checking, Savings, CD, IRA, Investment, Retirement) for ALL adult household members

___  Most recent two (2) years of tax returns (signed and dated) as submitted to the IRS and W2s/1099s.

___  If self-employed, provide a current 1099 and three (3) years of most recent signed tax returns as submitted to the IRS

___  Copies of ALL Mortgages and/or Warranty Deed

___  Copy of Homeowners Insurance (Declaration Page Only).  If uninsured, provide a completed Uninsured Affidavit​

___  Copy of most recent Mortgage Statement  

___  Copy of Mandatory Orientation Certificate
