The Company Approved Signature Form must be received by Consumer Affairs before we can accept affidavits from the Caregiver Company/License Holder or applicant. The notarized and signed affidavit will be accepted from the applicant along with their completed Caregiver ID Badge Application and required picture ID.

​The Palm Beach Board of County Commissioners approved a resolution on 3/12/19 that changed licensing fees for Home Caregivers.

The new fees are as follows:

  • Caregiver - Agency Affiliated with AHCA Clearance - $30

  • Caregiver requiring Fingerprinting - $100

 

AGENCY AFFILIATED APPLICANTS: 
New and renewing home caregiver agency affiliated applicants will no longer need an Agency Endorsement Affidavit form signed by the home care business in order for DCA to process the caregiver as "agency affiliated" to receive the reduced ID badging fee ($30).  Instead, the applicant will sign the Home Caregiver Consent & AHCA Background Authorization and Privacy Policy Acknowledgement form as part of the application process, which allows DCA to verify directly in the clearinghouse eligibility.
